New Patient Evaluation – $159

Re-Certification – $139
- Florida state law requires all patients to be reevaluated every 7 months by their doctor, and be issued a new certification.
Membership Plan – $24/mo
- Medical evaluation by Certified Physician
- Medical records review
- Includes all follow-ups and orders for the next 7 months
- Automatically renews monthly after 7 months
- Ease of automated payments
- One time $35 activation fee
New Patient Process

As per Florida Law, all initial evaluations need to be performed in person. To make the process as convenient as possible for our patients, our doctor will perform the initial evaluation directly in the comfort of your home or at a location of your preference. This approach allows you to receive professional, personalized care without the need to travel, making your experience simple, private, and stress-free.

After the initial evaluation is complete, our qualified physician will enter your information in the Medical Marijuana Use Registry and issue your Medical Marijuana Certification. You will then receive an email from the Florida Department of Health with instructions on how to apply for the Medical Marijuana ID Card.

Complete the application, pay the $75 (processing application fee) and then submit the application either online or by mail.

Once the application has been approved, you will receive an email with your temporary ID card, which you may then use to get your order filled at any Medical Marijuana Treatment Center (MMTC). You will also receive a physical ID card by regular mail.
No need to worry, we will guide you throughout the entire process!
Once you become our established patient, all subsequent Re-Certifications can be performed via Telehealth with our qualified physician.
